Comfort, dignity and care around your loved one

A familiar setting and a clear plan

Being at home can make daily routines easier for patients and families. Our team explains the agreed nursing tasks, shares observations and works with the existing medical plan.

Palliative care may begin alongside treatment for serious illness. When a person is approaching the end of life, care is discussed with the patient, family and treating doctor, with comfort and wishes at the centre of the plan.

How to arrange care

  1. Tell us the patient's location, condition and the support needed.
  2. Share the treating doctor's instructions, discharge summary or current care plan if available.
  3. We discuss the appropriate nursing tasks, schedule and team availability.
  4. The care arrangements are explained and confirmed before the first visit.
Answers for families

Frequently asked questions

Is palliative care only for the final days of life?

No. Palliative care can support a person with serious illness alongside other treatment. End-of-life care is planned when that stage becomes relevant.

What can a nurse help with at home?

Depending on assessment and medical instructions, a nurse may assist with personal care, positioning, observation and prescribed nursing tasks. We confirm the scope before booking.

Can you help with pain or other distressing symptoms?

Our nurses can observe and report symptoms, provide prescribed care within their role, and coordinate with the treating doctor. Decisions about medicines and changes to treatment remain with the treating clinician.

Can the family take part in the care plan?

Yes. We explain the practical arrangements and include the patient and family in discussions about daily support, respecting the patient's wishes.

Do you offer 24-hour care?

Tell us the schedule and level of support needed. We will confirm suitable arrangements and team availability before booking.

What information should I share before the first visit?

Please share the current care plan, medication list, relevant reports and the treating doctor's contact details when available, along with any immediate concerns.
